BSW Integrated Community Based Care Impact Report
Laying the foundation for transformation across Bath and North East Somerset, Swindon and Wiltshire
In April 2025, a major transformation programme began across Bath and North East Somerset, Swindon and Wiltshire (BSW), reshaping how community-based care is delivered. This Impact Report captures the first six months of that journey – a period focused on listening, learning, and laying the groundwork for lasting change.
Led by HCRG Care Group as system lead, the programme brings together NHS partners, local authorities, and the voluntary sector to create services that are more visible, equitable, and integrated – improving outcomes for the people and communities we serve.
Our approach: The 4D model of transformation
To guide this work, we’ve adopted the Design Council’s 4D methodology – a structured, evidence-based framework for delivering meaningful change. Each transformation project across BSW follows this four-phase process:
- Discover: Understand how services currently work and where the challenges lie.
- Define: Clarify the problems to solve and agree shared priorities.
- Design: Co-create and test new models of care with staff, partners and service users.
- Deliver: Implement and embed change, supported by a “test and learn” approach.
This inclusive and iterative approach ensures transformation is rooted in lived experience and shaped by those who use and deliver care.

- Key highlights and learnings from the Discover and Define phases
- The transformation priorities and enabling projects that will guide the next steps
- A roadmap for how we’ll move into design and delivery
