At HCRG Care Group, we are committed to improving health and care services across Bath and North East Somerset, Swindon, and Wiltshire (BSW). But to truly make a difference, we need your help. Whether you are a patient, a carer, or a local resident, you play a vital role in shaping the services that will support your community.
Together, we’re working towards a healthier future, where services are easier to access, more personalised, and focused on what really matters to you. We want you to access our services from day one, with ease and confidence. That’s why we’re creating a one-stop shop for all things community health—an integrated hub where you can find everything you need, from advice and information to support and care.
Your feedback is crucial in helping us understand your needs, concerns, and the types of care you want to see in your community. Through consultations, surveys, and direct conversations, we will be continuously listening and learning from you. By sharing your thoughts and ideas, you’ll be directly influencing the way we deliver care and improve outcomes for all.

A Phased Approach
We’re taking a phased approach to ensure a smooth and effective transition while engaging with the community, service users, colleagues and our system partners at every step. This will help us deliver services that truly reflect your needs and priorities.
Safe Transfer (By 1 April)
For 1 April, our immediate focus was on the safe transfer of services, ensuring they ran smoothly from day one. During this period, we engaged with key community representatives, as well as colleagues that were joining us, to provide clear communication about what’s changing, ensuring everyone felt informed and confident about accessing care.
First 100 Days
During the first 100 days, we are developing plans for improving and transforming services. As part of this, we are collecting feedback through surveys, consultations, and initial discussions with local groups, ensuring the foundation of our transformation plans reflects the community’s – and service user’s – voice.
First 6 Months
Next, we’ll start targeted engagement with the wider community, focusing on the priorities that matter most to you. This will include open forums, listening events, and outreach activities to ensure we hear from diverse voices across Bath and North East Somerset, Swindon, and Wiltshire.
Next 2 Years
Over the next two years, we’ll deliver the improvements outlined in our plans, using your feedback to guide the way. We’ll continue to work closely with the community, holding regular progress updates, celebrating achievements, and identifying new opportunities to improve.
We want to create services that truly benefit everyone – and that can only happen if we work together. So, whether it’s by taking part in surveys, attending local meetings, or providing feedback, your input will help us shape services that make a real difference to you, your family, and your community.
